Note: Nagar Bandhus residing outside India can become Member of Shri Vadnagara Nagar Mandal, Mumbai prescribed under FCRA (Foreign Contribution Regulation Act) 11(1) – Registration No: 08378133. We also appeal to Non Resident Indians to send Donations for various activities of Mandal.

Scan this Document along with the required Supporting Documents like : Copy of Salary certificate, Income tax return, Bank account statement etc. (In the case where there is no income other than interest from Fixed deposits, bank statement or a letter from the applicant stating his /her income may also be accepted.) and Documents from Check List given in the form.

The Medical aid disbursement will be guided by following guidelines
The guidelines are arrived at by considering following criteria
Nature of Disease
• Critical illness, life threatening disease like Cancer, Organ failure, Bye pass surgery etc.
• Accident or Disease resulting in permanent disability.
• Other diseases of less serious in nature.
  The appointed panel of doctors will decide the applicable category. The panel will have two or three doctors as appointed by the managing committee.
ECONOMIC BACKGROUND OF THE PATIENT (includes income of the Spouse and Son and unmarried daughter also if applicable)
• Category where monthly total income is less than INR 40,000/- per month.
• Category where total income is above INR 40,000/- but less than INR 60,000/- per month.
• Category where total income is above INR 60,000/- but not more than 80,000/- per month.

Criteria are being revised as and when felt necessary.
Necessary documents to be made available by the applicant.

* Copy of Salary certificate – Income tax return – Bank account statement etc.
* In the case where there is no income other than interest from Fixed deposits, bank statement or a letter from the applicant stating his /her income may also be accepted.

Note: DISBURSEMENT AMOUNT OF MEDICAL AID WILL BE DECIDED BY QUAMTUM OF ANY MEDICAL AID AVAILED FROM OTHER SOURCES INCLUDING THAT FROM HEALTH INSURANCE, THE QUALITY OF SERVICES USED AT THE HOSPITAL (I.E. THE CLASS OF ROOMS USED OUT OF VARIOUS CATEGORIES AVAILABLE.)

Caste Certificate for Mumbai Students from SVNM
For Cast Certificate to be issued to Student, Parents of Student OR Student have to be Members of Shri Vadnagara Nagar Mandal, Mumbai. If Parent of Student or Student is not Member then they will have to First Apply for Membership to obtain this Certificate.
This certificate is issued on specific request and for the purpose of applying for admission in Minority Quota.
All Information Required in the Form needs to be Filled and Submitted either by email on apps@shrivadnagaranagar.org
Incomplete Applications will not be allowed.
Certificate will be issued to Applicant within 5-6 Working Days after Receiving the Form.
This Certificate is issued for Mumbai Jurisdiction Only.
